Key Tasks and responsibilities:

Leadership within the Community Team, conducting oneself in a professional manner thus creating a consistent presence as a role model for everyone by ensuring that each individual is treated with the utmost respect and dignity in line with the mission, vision and values of the organization.

To complete any tasks assigned to high level of quality and best practice.

Responsible for own actions and those of staff that are supervised, to the organisation, accountable to themselves & the team.

An evidence trail must be maintained through documentation, and all must adhere and comply to all organisational policies and procedures.

Ensure staff are aware of individual supports, wishes & goals and that these are documented in the individual’s Good Life Plans.

Discuss concerns & issues as they arise, regular review & evaluation of effectiveness of meetingeach individuals supports & goals to ensure individuals are treated equally and in a non-biased manner.

Demonstrate through positiveleadership& positive approach with staff to motivate the team on a daily basis.

Promoting and ensuring the highest standards of professional support and safety in supportsand ensuring resources are applied efficiently.

Monitoring staff on a daily basis and supporting staff and individuals to ensure a high quality is maintained on a continual basis. This includes review of all documentation completed by staff and addressing any issues with staff members, recording these and report to manager.

To be proactive at team meetings, plan reviews scheduled, and all documentation completed from all reviews with any changes clearly recorded and staff informed of same

Induction process completed for all new staff members or new positions, ongoing observation & mentoring of staff working with individuals to ensure they receive appropriate supports to promote independence & achieving goals

Provides leadership and support to the team on current issues relevant in the area of Intellectual Disability.
Participation in service plan and objectives, continually inputting into the daily running of the service, promoting a positive approach to all supports & attempt to eliminate negativity.

Staff to be well motivated to ensure individuals receive positive self-directed supports at all times and staff team to be reminded that all interaction with individuals and families must be with a positive approach.

Support staff through regular on-site supervision & provide guidance on all documentation to be completed for this role.
Ensuring employees adhere to their professional code of conduct in line with their roles and responsibilities.

Actively listen to & motivate staff, be open minded to all ideas and suggestions and change, encourage & promote regular consultationwith staff.

Awareness of standards, framework and appropriate implementation within the service, monitoring best practice within the service & staff team. Participation in internalmonitors and improvement plans, be aware of responsibility in ensuring actions are implemented and monitoring of same.

Completion of regular house reviews, identifying areas forimprovement and following up that these are completed.

To continue to support individuals to promoteaspirations and future goals.

Team leaders to ensure all risk assessments are up to date, reviewed and communicated to staff regularly, ensuring any changes are monitored and reflected in supports.

Ensuring staff support each individual through SRV to live a self-directed life, ensuring the focus is on promoting choice and independence.

To actively seek new opportunities for the individuals in line with educational, employment and recreational establishments.

To facilitate new initiatives & activities that enhance & promote all independent skills, in line with the individuals plans, choices and wishes.

To develop all opportunities for individuals to utilise their independent skills and promote these at all times i.e. staff not ‘doing’ things for individuals that they are capable and competent to achieve themselves.

Participating in On Call roster as outlined in the On-Call procedures
Each Team Leader will participate in on-call as rostered.
Ensure staff are aware of the reasons to use on-call & what determines an emergency.
